Step 3: The Key Step - Submit Your Form to the Relevant Parties.

The key step now is to submit your form to the relevant parties. To ensure your bank, hospital, or institution is aware that you have revoked the previous power of attorney, you must inform them. Send them the new power of attorney or a notice of the cancellation of the old one.
